Insurance and Billing Instructions:
Check with your insurance company (call # on insurance card) to be sure Dr. Hughes is on the panel for your particular policy. Ask if preauthorization is required for Mental Health Outpatient visits. Ask about deductibles and copay.
You will be expected to pay your copay each visit. We will be glad to file your insurance for you.
Missed scheduled visits without 24 hour notice are charged to the patient at $125.00 each.
